No valid physical means no conditioning and no tryout.

The complete athletic packet must be completed, submitted, and approved before a player may participate.

Use the official DCHS and DCSS resources below. The full athletic packet—not only the doctor's physical page—must be completed.

Important: The Spanish GHSA physical form is only one part of the packet. The entire packet must still be completed. The DCSS page includes Spanish versions of many—but not necessarily every—required form.

Come prepared

What do I need for tryouts?

Players should arrive cleared, dressed to participate, and physically ready for a competitive soccer session.

Valid physical and complete packet

A player cannot try out without an approved physical and completed athletic packet. Open the English packet.

Running shoes

Wear secure athletic shoes suitable for running. Cleats are helpful but are not required until a player makes the team.

Weather-appropriate athletic clothes

Dress in layers that allow full movement. Avoid jeans or anything restrictive or unsafe for running.

Shin guards are helpful

Shin guards and soccer socks are useful during tryouts, but players do not need to buy them before making the team.

Hair secured; jewelry removed

Keep hair out of the face. Remove earrings, necklaces, watches, bracelets, and other jewelry before participating.

Arrive ready to begin

Be on time, dressed, and ready to participate. Players should be prepared to listen, compete, and respond positively to coaching.

Required personal medication

Bring any prescribed emergency medication you are required to keep with you, such as an inhaler, and follow school procedures.

No soccer ball or water bottle required

The program will provide soccer balls and water. Players only need to bring the items listed above.
